Instructions for filling out entry form:
1. This form uses Adobe Acrobat Reader
2. Use the "Tab" key to move from fill space to fill space
3. Click on down arrow on box to select division and class choices or use down arrow on keyboard
4. To print cutting lines for form, when in the Acrobat Reader print menu choose "Document and Mark Ups" from the Comments and Forms drop-down menu (upper right corner of print menu)
5. Print form(s) and cut apart. Print extra forms for future use if no information (other than date) will change.
